Credit Check Agreement

APPLICANT INFORMATION :

CONSENT TO PERFORM CREDIT CHECK :

I, the undersigned, hereby authorize CJO Property Solutions to obtain and review my credit report for the purpose of evaluating my rental application. I understand that this information will be used solely for the purpose of determining my eligibility as a tenant and will be kept confidential.

I acknowledge that:
1. CJO Property Solutions will request and review my credit report from one or more consumer reporting agencies.
2. The credit report will include information about my credit history, including, but not limited to, my credit score, payment history, and any public records or collection items.
3. This authorization is valid for a period of 90 days from the date of signature.

Release of Information:
I release CJO Property Solutions and its employees, agents, and representatives from any liability arising from the use of the information obtained in my credit report.

APPLICANT's SIGNATURE AND DATE

By clicking the box "I agree' and providing your electronic signature, you consent to use electronic records and signatures for this transaction. You understand that your electronic signature is legally binding, and you acknowledge that you have the option to sign this document in a non-electronic form. This process complies with the E-SIGN Act and other relevant regulations. Your electronic signature and associated records will be retained securely for your reference.
